01. Evidence
Half the bank is a stem that already contains the picture — your only job is to name it.
A quarter of the bank rewards one-line facts you either know cold or have no path to.
Negation and "all of the above" barely appear — treat them as noise, not as a strategy.
Option keys run close to even across A to D, with E practically unused, so no positional shortcut survives. A slice of the bank carries no recorded key — one sitting in full, plus strays from the others.
02. Anatomy
›Rule
If no task sentence → read options first (diagnosis-implied). Else read last line → pivots → predict → options.
03. Families
›01 Recognition — Name It
›Diagnosis — stated
The stem hands you a complete clinical picture and asks for the disease name outright.
[Age/sex] + [presenting complaint] + [2–4 discriminating findings] + "most likely diagnosis?"
The discriminator is almost always a modifier inside the stem — the word that flips the answer: sudden vs gradual, painless vs tender, site (DIP vs MCP), timing (weeks after sore throat vs during URTI).
Options are sibling diseases in the same system. You lose by picking the sibling that matches half the stem. Name all four options first, then find the one line that separates them.
- Read only the last sentence — confirm the task is "name the disease".
- Write the 3 findings that matter most; ignore age and background noise.
- For each option, ask: does it explain all three? One will. That is the answer.

›Diagnosis — implied
No question sentence at all. The stem is a bare vignette and the options are four disease names. The task is still diagnosis.
[Age/sex] + [finding] + [finding] + [finding] — then straight into options.
No explicit ask means the option set defines the task. Read the options before the stem if the stem has no question mark — that is the whole trick to this type.
These feel ambiguous and slow you down. They are not harder than stated diagnosis; they are just missing the instruction line.
- If there is no question sentence, read the four options first — they tell you what is being asked.
- Identify what the vignette is pointing at (organ system, then process).
- Apply the same discriminator test as any diagnosis question.

›02 Facts — State It
›One-line recall
Short factual stems with no vignette. Pure retrieval: highest-yield facts, one per question.
"Most common site of X?" / "X is caused by?" / "Normal value of Y?"
The only discriminator is whether you know the fact. No clinical reasoning available.
Distractors are neighbouring facts — other sites, other causes, other values. You either own the fact or you do not.
- Recognise instantly that there is nothing to reason through — it is memory or nothing.
- Attach the fact to a hook (a rule, a mnemonic, a comparison) so it survives.
- If you did not know it, mark it in the error log — this is the cheapest kind of mark to recover.

›Recall — worded as a fact question
Same retrieval demand, phrased as "cause of", "which of the following is true", "most widely used".
"Cause of X?" / "All of the following are true about X except" / "Drug of choice for X?"
The discriminator is the specificity of the fact. Broad true statements lose to narrow precise ones.
When several options are technically true, the setter wants the most specific / most characteristic one. Rank them, do not scan them.
- Accept that multiple options may feel true.
- Pick the one that is most characteristically true — the one you would quote in clinic.
- Convert to a flashcard immediately; this type is pure accumulation.

›Site / location recall
"Most common site of X", "which organ/nerve root/level is affected" — the location is the answer.
Condition given → location asked, or vice versa.
You either hold the canonical site (osteosarcoma lower femur, diverticulae sigmoid) or you do not.
Options are usually four anatomical sites in the same region — plausibility does not help, only the memorized fact does.
- Name the canonical site before reading options.
- Match; if two options feel close, it is a sub-site distinction you must know cold.
- Log it as a fact gap, not a reasoning gap.

›Test / investigation selection
Given the patient, pick the investigation. The qualifier in the stem decides whether you want the cheap screen or the gold standard.
[Vignette] + "best initial test / investigation of choice / test that confirms?"
In the qualifier. "Initial" means cheapest and least invasive; "confirms" means the reference standard. Same patient, different correct answer.
Answering with the confirmatory test when the stem said "initial" (or vice versa). Read the qualifier before the medicine.
- Underline the qualifier: initial / confirm / gold standard / next.
- Ask what the exam wants — a screen or a proof.
- Eliminate any test that is right but answers a different question than the one asked.

›Management / first-line treatment
Given the diagnosis (stated or implied), pick what you do next. Includes drug of choice, antidote, regimen, surgical versus medical.
[Vignette] + "most appropriate treatment / management / drug of choice / antidote?"
The severity line. Stable versus unstable, mild versus severe, obstructed versus not — that word picks the answer.
Options split into "definitive" and "supportive" treatments. The exam usually wants the definitive one unless the patient is unstable, in which case it wants resuscitation.
- Find the severity word first (unstable, severe, acute, shocked).
- If unstable, the answer is almost always the resuscitation option, not the definitive one.
- Then pick the definitive treatment for that severity.

›Next step in management
Not "what is the diagnosis" and not "what is the treatment" — what do you do next, in sequence.
[Vignette] + "next step in management / most appropriate next step?"
The chronology. Where are we in the algorithm — before or after the confirmatory test?
Jumping to the final treatment when the algorithm demands an intermediate step (test before treating, stabilise before imaging).
- Write the algorithm for the condition in three steps.
- Locate the patient on it — what has been done, what has not.
- Pick the next node, not the destination.
A dog bite wound is cleaned with povidone iodine. What is the next step?

›Contraindication / safety
Given a condition, procedure or drug, pick what must not be done.
"Which is a contraindication for X?" / "which should be avoided in Y?"
The intersection of two facts — the condition and the intervention.
Options are other true statements about the intervention, just not contraindications.
- List what the intervention is contraindicated in.
- Match against the vignette; ignore options that are otherwise true but unrelated.

›04 Explanation — Why It Happens
›Mechanism / pathophysiology
Asks why something happens: which pathway, which vessel, which substance, which tract.
[Finding] + "mechanism / responsible for / which area is affected / where do fibres end?"
The link between the finding in the stem and the underlying process. You must know the mechanism, not the label.
Distractors are real structures and real processes from neighbouring systems — plausible to anyone without the mechanism.
- Translate the clinical finding into its physiological consequence.
- Trace that consequence forward: which structure, which pathway, which tract.
- Pick the option that the chain actually reaches.

›Organism identification
Given a presentation, culture, stain or serology, name the organism.
[Presentation/stain/culture] + "most likely causative organism?"
The specific clue — the Gram stain shape, the culture medium, the vector, the immune status.
The common organisms of the same syndrome. The answer is the one matching the clue, not the one matching the syndrome.
- Isolate the discriminating clue first (stain, culture, geography, host status).

›Drug fact / antidote / adverse effect
Which drug, which antidote, which adverse effect. Pharmacology tested as one-liners wired to a clinical hook.
"[Drug] causes X due to which adverse effect?" / "antidote for X?" / "drug of choice for X?"
The single drug-to-effect pair. There is no reasoning path — only recall.
Options are the real adverse effects of the same drug class or the same drug. Only the specific pair counts.
- Name the class effect list in your head.
- Pick the pair the stem actually describes.
- If the antidote is asked, recall mechanism — the antidote always blocks the mechanism.

›Complication / sequel
Given a disease or procedure, name the commonest or most important complication.
"[Condition/procedure] — most common complication?"
The known natural history of the condition.
Distractors are dramatic but rare complications; the answer is the boring common one.
- Recall natural history, not possibility.
- Pick the commonest unless the stem says "most important" (then pick the lethal one).

›05 Readers — Locate & Interpret
›Anatomy localisation
Which nerve, muscle, foramen, tract, level. Given a deficit or a lesion, find the structure.
[Lesion/injury] + "which nerve is injured / what exits this foramen?"
The deficit described. Translate the deficit into a root, nerve, or level, then match.
Options are adjacent nerves from the same plexus or limb — the discriminator is the specific motor or sensory pattern.
- List what the structure does: motor, sensory, reflex, territory.
- Match the deficit pattern in the stem against those functions.
- Choose the nerve whose territory matches completely — partial match is a trap.

›Data interpretation / calculation
Labs, gradients, ratios, percentages. The stem hands you the numbers and asks you to interpret them.
[Values given] + "what does this indicate / which diagnosis / calculate X?"
The arithmetic or the threshold. SAAG, anion gap, sensitivity, odds, BMI.
You do not need to be slow — you need to know which formula applies and apply it once.
- Identify the quantity being asked for.
- Recall the single formula or threshold for it.
- Apply it; the diagnosis then follows from the interpretation.

›Negation — EXCEPT / NOT
Ask for the false statement, the least likely, the one that is incorrect.
"All of the following are true about X EXCEPT?"
You must know the truth of four separate claims — the hardest type, and deliberately rare here.
Reading it as a normal question and answering the most-true-looking option, which is the trap.
- Stop. Rewrite the task as "find the false one".
- Verify each option independently — this type is slow by design.
- Only then answer.

›Direct comparison
Ask you to distinguish two conditions on one discriminating feature.
"Distinguish A from B on the basis of…"
The single differentiator between the two — CSF, histology, distribution, timing.
Options are real differences but not the asked one (e.g. aetiology asked, given a symptom difference).
- Write the classic differentiator table for A vs B.
- Match the question to the row of the table it asks about.
- Answer with that row only.

04. Option Patterns
›Seven patterns
- Sibling → find pivot
- Category → check task
- Modality → initial vs confirmatory
- Lookalike → own difference
- Severity → match word
- Supportive/Definitive → unstable→resuscitate
- True-but-irrelevant → answer asked task
05. Solve
›1. Task
Read the last line first and name the task in three words — diagnosis, test, management, next step, mechanism, recall. The task decides which memory you search. If there is no task line, read the options first.
›2. Pivots
Find the two or three findings that separate the options, not the whole story. Ignore age, sex and background unless one of them is the discriminator.
›3. Predict
Say the answer in words before you read the options. Four seconds. It stops you accepting the first plausible option.
›4. Match
Look for your prediction in the option set. If it is absent you have misread the task or do not know the fact — go back to step one before eliminating.
›5. Eliminate
Remove options answering a different task, then wrong-category options, then the sibling that fails one pivot. Three usually fall at once.
›6. Commit
No negative marking. Never leave a blank. If two survive, take the more specific and move on.
